These Terms of Use ("Terms") govern your use of azinjurydoctors.com, our phone lines, and our text messaging program (together, the "Services"), operated by AZ Injury Doctors ("AZ Injury Doctors," "we," "us," or "our").
By using this website, calling us, texting us, or submitting a form, you agree to these Terms. If you do not agree, please do not use the Services.
The short version
- This site helps people injured in Arizona find and coordinate care. It is not medical advice and it is not legal advice.
- If you are having a medical emergency, call 911. Do not use a form.
- Texting is optional. Reply STOP to stop, HELP for help. Message frequency varies; message and data rates may apply.
- Some calls are answered by an AI assistant. Ask for a person and one will take over.
- Tell us the truth on our forms, and only send us information about yourself or someone you are authorised to act for.
- How we handle what you send is covered by our Privacy Policy.
Who we are and what this site is
AZ Injury Doctors coordinates care for people injured in accidents across Arizona. We help you find an appropriate provider in our statewide network and help arrange the next step.
We are a care coordination service. The providers in the network are independent. When you are treated, you are the patient of that provider, and your care, your medical record and your relationship are with them. Nothing on this site creates a relationship between you and a provider until that provider accepts you as a patient.
This site is not medical advice
Everything on this website — service descriptions, symptom information, articles, answers to questions — is general information. It is not a diagnosis, not a treatment plan, and not a substitute for being examined by a clinician who knows your history.
Submitting a form, calling us, or texting us does not create a doctor–patient relationship. Do not delay seeking care, and do not disregard advice you have already been given, because of something you read here.
If you think you are having a medical emergency, call 911 or go to the nearest emergency department. Our intake line is answered around the clock, but it is not an emergency service and it is not monitored as one.
This site is not legal advice
AZ Injury Doctors is not a law firm and does not practise law. We work with personal injury attorneys on behalf of patients who ask us to, but nothing on this site is legal advice, and using the Services does not create an attorney–client relationship with anyone.
We make no promise about the outcome, value or timing of any insurance claim or legal matter. Questions about a claim belong with your attorney.
Costs and payment
Nothing on this website is a quote, a guarantee of coverage, or an agreement about payment. Any financial arrangement for treatment is made directly with the treating provider, in writing, and its terms are theirs and yours — not ours. If you are not sure what something will cost you, ask before you are treated.
Text messaging program terms
These terms apply to the AZ Injury Doctors text messaging program. They sit alongside the SMS section of our Privacy Policy, which describes how we handle the information involved.
Program description
The AZ Injury Doctors messaging program sends two kinds of message, and you choose which you receive:
- Care coordination and customer care — appointment reminders and confirmations, scheduling changes, intake follow-up, and replies to questions you have asked us.
- Marketing and informational — occasional updates about services available through the network. You receive these only if you have separately agreed to them.
Eligibility and consent
You must be 18 or older and the account holder of the mobile number you give us, or a person authorised by the account holder to consent on their behalf. Do not submit someone else's number without their permission.
Consent to receive text messages is optional. It is never required in order to use this website, submit a form, get a call back, or receive care, and the consent box is never pre-checked for you.
Message frequency, rates and delivery
Message frequency varies — it depends on where you are in the intake and scheduling process. Message and data rates may apply, according to the plan you have with your mobile carrier.
Mobile carriers are not liable for delayed or undelivered messages. Delivery depends on your carrier and your device, and we cannot guarantee that any particular message will arrive.
How to stop
Reply STOP to any message from us at any time. You will receive one confirmation message and then no further messages from that program. You can also call 602-753-8818 or tell any member of our team, and we will remove you.
Opting out of texts does not cancel an appointment and does not stop us reaching you by phone about care you have asked for. If you want us to stop contacting you altogether, say so and we will.
How to rejoin
If you have opted out and want texts again, reply START or UNSTOP to the same number, submit a form with the consent box checked, or ask us on a call.

Industry standards and legal compliance
We operate this program in line with CTIA messaging principles and best practices and with applicable carrier requirements, and we comply with the Telephone Consumer Protection Act and other applicable federal and Arizona law. We honour opt-out requests promptly however they reach us — a reply, a call, an email or a word to a member of the team all count.
Phone calls and our AI assistant
Some calls to our published numbers are answered by an AI-powered voice assistant, which identifies itself as automated at the start of the call. You can ask for a person at any point and one will take over. Calls may be recorded or transcribed; if you would rather not be recorded, say so at the start of the call.
Where an outbound call uses an artificial or prerecorded voice for a marketing purpose, we place it only with your prior express written consent, which you may revoke at any time. The Privacy Policy covers what happens to recordings and transcripts.
Using this site
You may use the Services for your own personal, non-commercial purposes, or on behalf of a patient, a client or a referral you are authorised to act for. You agree not to:
- submit information that is false, or that belongs to someone who has not authorised you to submit it;
- use the Services to send unlawful, abusive, harassing or misleading content;
- scrape, harvest, crawl or bulk-collect anything from the site by automated means;
- attempt to gain access to any account, system or data you are not entitled to;
- interfere with the operation of the site, or introduce anything malicious into it;
- copy, republish or resell our content, or use it to train a commercial model, without our written permission;
- use the Services to solicit our patients, our network providers or our referral partners.
We may suspend or refuse access to anyone who does any of the above.
Information you send us
Tell us the truth. Intake decisions and scheduling are made on what you tell us, so inaccurate information can delay or misdirect your care.
Only send what belongs to you, or what you are authorised to send. Referring providers and law firms who submit patient information through our forms confirm that they have the authority to do so.
Do not send complete medical records, imaging studies, insurance card images or Social Security numbers through the general contact form or by ordinary email. Neither is a secure channel. Call 602-753-8818 and we will tell you the right way to send them.
By submitting information, you permit us to use it to respond to you and to coordinate your care, as described in the Privacy Policy. You keep ownership of what you send.
Our content and marks
The text, design, layout, graphics and code of this website are owned by AZ Injury Doctors or used under licence, and are protected by copyright and other laws. The AZ Injury Doctors name and logo are our marks. You may read, print and share pages for your own use; anything beyond that needs our written permission.
Every photograph on this site is stock imagery used for illustration. No image depicts an actual patient, an actual provider, or a facility we own.
Other websites and services
Our site links to and relies on services operated by other companies — our forms, our messaging platform, our analytics. We do not control them and we are not responsible for their content, their availability or their practices. When you leave our site, the terms and privacy policy of wherever you land apply, not ours.
Availability
We aim to keep the site up and our intake line answered around the clock, but we do not guarantee that the Services will be uninterrupted, error-free or available at any particular moment. We may change, suspend or discontinue any part of the Services at any time.
Disclaimers
The Services are provided "as is" and "as available", without warranties of any kind, whether express or implied, including any implied warranties of merchantability, fitness for a particular purpose, non-infringement, accuracy or availability, to the fullest extent the law allows.
We do not warrant that information on this site is complete or current, that the site is free of errors or harmful components, or that any message we send will arrive. We do not warrant the care, competence or availability of any independent provider in the network — providers are independent, and their professional judgment is their own.
Limitation of liability
To the fullest extent the law allows, AZ Injury Doctors and its owners, officers, employees and contractors are not liable for indirect, incidental, special, consequential, exemplary or punitive damages, or for lost profits, lost data, or loss of goodwill, arising from your use of the Services — even if we were told such damages were possible.
To the fullest extent the law allows, our total liability for any claim relating to the Services is limited to one hundred US dollars ($100).
Nothing in these Terms limits liability that cannot be limited by law, including liability for fraud, for personal injury caused by negligence where that limitation is not permitted, or for anything else Arizona law does not allow to be excluded. Nothing here limits or affects any claim you may have against a treating provider.
Indemnification
You agree to indemnify and hold harmless AZ Injury Doctors and its owners, officers, employees and contractors against claims, damages, losses and reasonable legal fees arising from your breach of these Terms, from information you submit that you were not authorised to submit, or from your misuse of the Services.
Governing law
These Terms are governed by the laws of the State of Arizona, without regard to its conflict-of-laws rules. You and AZ Injury Doctors agree that any dispute relating to the Services will be brought in the state or federal courts located in Arizona, and each of us consents to the jurisdiction of those courts.

Changes to these Terms
We may update these Terms as the business, the law or our technology changes. The effective date at the top always tells you which version you are reading, and a material change will be flagged on this page. Continuing to use the Services after a change means you accept the updated Terms.
General
If any part of these Terms is found unenforceable, the rest stays in force and the unenforceable part is applied as closely to its intent as the law allows. Our not enforcing a provision is not a waiver of it. These Terms, together with the Privacy Policy, are the entire agreement between you and us about the Services. You may not transfer your rights under these Terms; we may transfer ours as part of a sale or reorganisation of the business.
