This Privacy Policy explains what information we collect through this website, our phone line and our text messaging program, how we use it, and the choices you have.
By using this website, calling us, or texting us, you agree to this Privacy Policy.
The short version
- We collect what you tell us — your name, phone number, email, city and a description of what happened — so a coordinator can call you back and help you find care.
- We do not sell your personal information, and we do not rent or trade it.
- We never share your mobile number or your text messaging consent with anyone for their own marketing.
- You can stop hearing from us at any time. Reply STOP to any text, use the unsubscribe link in any email, or call 602-753-8818 and ask.
- This website is not the place to send medical records. Call us and we will tell you where they should go.
Who this policy covers
This policy applies to information we collect through:
- this website, including every contact, callback, referral and intake form on it;
- calls to and from 602-753-8818 and any other number we publish;
- text messages sent to or from our business numbers; and
- email sent to the addresses we publish.
It does not apply to the separate privacy practices of the independent providers in our network, of any law firm you work with, or of any other website we link to. Once you are a patient of a provider in the network, the medical record that provider creates is governed by that provider's Notice of Privacy Practices under HIPAA, not by this policy.
Information we collect
Information you give us
When you fill in a form, call, or text us, we may collect your name, mobile or landline number, email address, city or ZIP code, the date of your accident, a description of your injuries or symptoms, whether you have already received care, and the name of your attorney or law firm if you have one. Referring providers and law firms may also submit patient contact details, dates of birth, dates of loss, symptoms, prior treatment and supporting documents through the forms built for them.
Information collected automatically
Like most websites, ours records technical details when you visit: IP address, browser and device type, operating system, the pages you view, the page that referred you and the time of your visit. We use cookies and similar technologies for this — see Cookies and analytics below.
Information from others
A law firm, a referring provider, or a family member acting on your behalf may give us your contact details so that we can reach you about care.
What we ask you not to send
Please do not send complete medical records, imaging studies, insurance card images or Social Security numbers through the general contact form or by ordinary email. Neither is a secure channel. Call 602-753-8818 and we will tell you the right way to send them.
How we use your information
We use the information above to:
- respond to your call, text, form or email, and follow up if we miss you;
- understand what happened and what kind of evaluation or care may be appropriate;
- help identify and coordinate an appointment with a provider in our Arizona network;
- coordinate with your attorney or your referring provider when you have asked us to;
- send appointment reminders, confirmations and care-coordination updates;
- send marketing or informational messages, only if you have asked to receive them;
- answer questions, resolve problems and improve how this website and our intake work;
- keep records, prevent fraud and abuse, and meet our legal obligations.
We do not use your information to make automated decisions that produce legal or similarly significant effects.
Text messages (SMS) and your consent
We operate a text messaging program so that our coordination team can reach you quickly after an accident. Here is exactly how it works.
How you opt in
You opt in by checking the SMS consent box on one of our forms, by texting us first, or by telling a member of our team on a call that you want text updates. Consent to receive text messages is never required in order to use this website, submit a form, or receive care, and the box is never pre-checked for you.
What we send
Two kinds of message, and you choose which:
- Care coordination and customer care — appointment reminders and confirmations, scheduling changes, intake follow-up, and answers to a question you have asked us.
- Marketing and informational — occasional updates about services available through the network. You receive these only if you have separately agreed to them.
Message frequency varies. Message and data rates may apply.
How you stop
Reply STOP to any message to opt out. You will receive one confirmation message and then nothing further, unless you opt in again. Reply HELP for help, or call us at 602-753-8818. You can also ask any member of our team to remove you.
Mobile carriers are not liable for delayed or undelivered messages.
Your mobile information and your consent stay with us
No mobile information will be shared with third parties or affiliates for marketing or promotional purposes. Information sharing with subcontractors in support services, such as customer service, is permitted. All other use case categories exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
In plain English: the phone number you give us, and the consent you give with it, are used by AZ Injury Doctors to contact you. They are never sold, never rented, never passed to another company for that company's marketing, and never used to build a list anybody else can buy. The only parties who touch them are the vendors who deliver the messages for us — our messaging platform and the telecommunications carriers — and they may use them only to send the message.
Our Terms of Use set out the messaging program itself — eligibility, how to rejoin after opting out, and the industry standards we hold it to.
Phone calls, recording and our AI assistant
Calls may be answered by an automated assistant. Some calls to our published numbers are answered by an AI-powered voice assistant that can take your details, answer common questions and route you to a person. The assistant identifies itself as automated at the start of the call. You can ask for a person at any point and one will take over.
Calls may be recorded or transcribed for quality, training, accuracy of intake and record-keeping. If you would rather not be recorded, tell us at the start of the call and we will take notes instead, or contact us by form or email.
Automated and prerecorded outbound calls. We place outbound calls to return your enquiry and to coordinate care. Where a call uses an artificial or prerecorded voice for a marketing purpose, we place it only with your prior express written consent, which you may revoke at any time by telling us on the call, replying STOP to a text, or calling 602-753-8818.
Recordings and transcripts are treated as confidential and held under the same rules as everything else in this policy.
How we share information
We share your information only in these situations:

We do not sell your personal information. We do not share it with third parties or affiliates for their own marketing or promotional purposes. We do not permit our service providers to use it for their own purposes. And as stated above, mobile information and text messaging consent are excluded from sharing entirely.
Cookies and analytics
We use cookies and similar technologies to keep the site working, remember your preferences, and understand in aggregate how visitors use the pages — which are read, which are not, and where people get stuck. This is measurement, not surveillance.
You can refuse or delete cookies in your browser settings; some parts of the site may not work as well if you do. Where we are required to honour a Global Privacy Control signal from your browser, we do.
We do not allow advertising trackers to transmit information about your health, your symptoms or the care you are seeking to advertising networks.
Health information and HIPAA
This is worth being precise about.
This website is an intake and information channel, not a medical record. When you tell us about your accident through a form, a text or a call, you are giving that information to AZ Injury Doctors so that we can help you find care. We protect it, and we limit who sees it, as described in this policy.
Once you are treated, HIPAA applies to your treating provider. The medical record created by a provider in our network is protected health information held by that provider, and it is governed by that provider's own Notice of Privacy Practices, which they will give you. Your rights of access, amendment and accounting under HIPAA are exercised with the provider who holds the record.
Nothing on this website is medical advice, and submitting a form does not create a doctor–patient relationship. If you are having a medical emergency, call 911.
How long we keep information
We keep contact and intake information for as long as we need it to coordinate your care and to keep an accurate business record, and after that for as long as the law requires or allows — which, for records connected to an injury claim, can be several years. Records of your text messaging consent, and of any opt-out, are kept for as long as we operate the messaging program, because we are required to be able to show them. When information is no longer needed we delete it or de-identify it.
How we protect information
We use administrative, technical and physical safeguards appropriate to the sensitivity of what we hold: encryption of the website in transit, access limited to team members who need it, and vendors selected for their own security practices. No method of transmission or storage is perfectly secure, and we cannot guarantee absolute security — which is the reason for the request above not to send records or Social Security numbers by ordinary email or through a general web form.
Your choices
- Text messages — reply STOP to opt out, HELP for help.
- Email — use the unsubscribe link in any marketing email, or ask us.
- Calls — tell us on the call, or ask us in writing, and we will stop calling for marketing purposes.
- Cookies — control them in your browser.

Opting out of marketing does not stop the messages we need to send to coordinate care you have asked for. If you want those to stop too, tell us and they will.
State privacy rights
Depending on where you live, you may have additional rights — to know what we collect, to access a copy, to correct it, to delete it, to opt out of sale or of targeted advertising, and not to be discriminated against for exercising any of them.
We do not sell personal information, and we do not use it for targeted advertising or for profiling that produces legal or similarly significant effects.

We will verify who you are before we act — usually by confirming details we already hold — and we will respond within the time the applicable law allows. An authorised agent may make a request on your behalf with your written permission.
Children's privacy
This website is intended for adults. We do not knowingly collect information from children under 13. Where a parent or legal guardian contacts us about an injured minor, they are giving us that minor's information themselves, on the minor's behalf, and we treat it with the same care as everything else here. If you believe a child has given us information without a parent's involvement, call 602-753-8818 and we will delete it.
Other websites
Our site may link to other websites — a provider, a law firm, a resource we think is useful. We do not control them and we are not responsible for their privacy practices. Read their policies before giving them information.
Changes to this policy
We may update this policy as the business, the law or our technology changes. The effective date at the top always tells you which version you are reading, and a material change will be flagged on this page. Continuing to use the site, or continuing to receive our messages, after a change means you accept the updated policy.
